A thinner, grim-looking Tracy Morgan is back home in Cresskill, New Jersey, after spending more than a month in a hospital and rehab facility following a harrowing crash on the New Jersey Turnpike.

The Post caught this first post-accident glimpse of the funnyman on Saturday, confirming another milestone in his brave fight to recover from a collision that killed his friend and fellow comic James “Jimmy Mack” McNair.

The “30 Rock” star is still using a wheelchair, and his left leg is bandaged — but pals unloaded a walker into his home, and a therapist paid a 45-minute visit on Saturday morning, which was his first full day home following his release on Friday.

“Tracy has been released from the rehabilitation center and will continue his recovery efforts at home with an aggressive outpatient program,” his publicist, Lewis Kay, confirmed. “He asked me to pass along his sincerest gratitude to everyone who has helped him get to this point.”

Morgan, 45, who lives with his fiancée, model Megan Wollover, and their year-old daughter, Maven Sonae, suffered several cracked ribs and a badly broken leg before being moved to inpatient rehab on June 20.

Morgan was returning from a stand-up gig at a Delaware casino on June 7 when his limo van was slammed from behind by a Walmart tractor-trailer.

Officials are blaming the crash on truck driver Kevin Roper, 35, of Jonesboro, Georgia.

Roper was driving 20 mph over the speed limit, had fallen asleep at the wheel and had been awake for more than 24 hours before the crash, according to the assault and death-by-auto charges against him. Those allegations are repeated in a negligence lawsuit filed against Roper and Walmart this week by Morgan and three other injured passengers.
