Health inspectors have shut Eli Zabar’s E.A.T. Gourmet shop on the Upper East Side.

“E.A.T. has a serious mouse infestation as well other food safety deficiencies,” the city Department of Health and Mental Hygiene said in a statement today.

“They will be closed temporarily until they can address these problems.”

A customer took video of a giant rat scurrying around the shop’s breadbaskets on Sunday.

Until the incident, E.A.T. had an “A” rating from the Health Department.

But the shop, at 1064 Madison Ave., has a history of problems, city records show.

An inspection in October found evidence of roach and fly infestation in the store. Another inspection in June found evidence of mice.

“We are under reconstruction at the moment,” said a worker who answered the phone at the store this afternoon.
